Through the Texas HCS and TxHmL programs, we support where you live, how you spend your days, and the health and professional care that keeps you well. Every service follows your person-directed plan, built with you, your family, and your service coordinator.

HCS lets you choose where you live. Wherever that is, we provide the daily support written into your plan, so home feels like yours.

Share a small home in the community with no more than three housemates. Trained staff help with daily routines, meals, medications, and getting out and about, while you make the choices that shape your day.
Support in your own home or your family's home, including transportation. Staff help with personal care, cooking, cleaning, shopping, and appointments, and teach the skills that build independence.
Through Community First Choice, people who live in their own or family home can get hands-on help with daily activities and coaching to learn new skills, such as dressing, meal prep, money, and using the bus.
Days filled with things that matter to you: learning, friends, work, and time out in the community. And a real break for the families who love you.
Daytime activities at community sites chosen around your interests, from classes and volunteering to outings and hobbies. You build skills, confidence, and friendships in the places other people go every day.
Help figuring out what kind of work fits you, finding and applying for a job, and on-the-job support to learn the role, keep it, and grow in it. Real work, real pay, alongside coworkers.
Short-term support, in your home or out of it, that gives family caregivers time to rest, run errands, or simply recharge, knowing you are safe and well supported.

Therapy for people with a brain injury that helps rebuild memory, attention, problem solving, and everyday thinking skills.

We work closely with your primary care doctor, dentist, therapists, behavior analysts, and LIDDA service coordinator, so everyone supporting you is on the same page.
TxHmL is for people who live in their own home or their family's home. It adds essential services to the support you already have, so life can keep happening right where it is.
